Question: During An Eye Exam, When Should They Do The Visual Field? I went to VisionWorks today and they did the Visual Field first... Shouldn't they do that closer to the end?
Answer: It doesn't matter what order it is done in at all. All that matters is that it is done. The results will be the same whatever time you do it. All that it measures is your peripheral vision, and this will not change from the beginning to the end of your test
Question: Eye problems, visual field etc? Ok, starting a few years ago, I had these regular losses of visual field, especially when stressing. Its like i see nothing there. Where there is supposed to be an eye on people, I see skin.
Last time I had it was almost a year ago, but tonight I got it again. What happens is: after the visual field is regained, I often feel pain just behind my right eye. Also, this year, I have noticed that when im out in the sun, I see diffrently with my eyes. One eye has a brownish tone, the other has a blueish tone.
I have read about Glaucoma, and some of the symptoms are correct. But Glaucoma is a disease people get at age 40+, and it is usually people with a history of it in the past. It doesnt say anything about it, but could this be caused by staring at my computer for longer periods? In the last couple of weeks, ive been in front of my computer for hours. i am usually on for 3 hours+, but the last 2 days, ive been on the computer for more that 5 hours every day because of exams.
anyone know if this has anything to do with Glaucoma? And if it isnt, anyone know what it is?

Answer: Loss of vision for 20 to 60 minutes followed by a one sided headache (pain behind the eye) after the vision returns is classic migraine. None of the symptoms match glaucoma.
See your family doctor to discuss migraine treatment.
Question: What is the accuracy rates or statistics of a VISUAL exam diagnosing Dermatofibroma and it being correct? I have a total of three knots currently. One on my right arm, one on my right buttock, and one on the external portion of the vagina slightly above the clitoris. My mother, grandfather, and aunt have all had melanoma cancer. Since the knots were concerning (light brown in color few slight speckles of black but nothing major on the buttocks and arm and one on vagina is skin toned - one on arm and buttocks size of a dime and one on vagina size of a b b - all somewhat of an odd circular shape) I went to the dermatologist after a friend (doctor) at cancer research suggested doing so. The Dermatologist visually diagnosed the one on my arm and buttocks (didnt mention the vagina as I thought it might be from shaving or something like that and gynecologist apt the following day) as dermatofibroma . During the exams there was a spot found on my back (upper right side near the arm pitt area) which was biopsied using the shave technique. The biopsy came back positive for melanoma and referred me to a surgeon which I see Friday 2/12/10 for initial over view I would assume and date of expected operation. The wound stung and began to tingle around it and after a few days the tingle is now up my shoulder. I called the dermatologist and didnt get a call back for several hours and since I work in the ER I asked a Dr for advice (not sure WHAT was going on). She looked to see if it were infected thinking maybe thats pushing on a nerve, it wasnt. She said she believes during the biopsy he may have damaged a nerve. The dermatologist finally called back and told me if its not infected (I informed him the ER dr said its not) then its a coincidence and has nothing to do with the biopsy and cancer spot at all. (Tingling still there and NO ANSWERS AS TO WHY other than from the ER dr and what she believes- has anyone had this? I would assume its rare but it WONT GO AWAY!) I then informed him during the phone call of the lump in the vaginal area because my Gyno was stumped saying "maybe its a form of skin tag. Watch it and let me know if it changes and if so we certainly need to remove." The Derm said "its likely dermatofibroma." I asked if that was common and he said NO and I asked if he had ever had a patient with it and he said NO and after looking it up NO!!! The Derm won't biopsy the three lumps to make an OFFICIAL pathologist reported diagnosis for the dermatofibroma insisting there is no point because they will just come back. My ER dr who has had melanoma was furious he left the knots on (she seen the one on arm and buttocks) and insisted, her exact words, "HE IS A QUACK!." She insists I consult the surgeon and insist on removal of ALL the lumps. I have no idea whats going on, and the derm didnt really tell me anything but I have to see a surgeon, the name, and number. What do I expect? Can I insist on these being removed as well or at least if I express my concern is it an option at that point or do I need? I am getting laid off in a month and my insurance will be gone till I find new employment (which im looking) and get more insurance. Surgery will be soon and there are no other Dermatologists who can get me in soon for a second dermo opinion, earliest being May!!! So how accuarte are dermatologists with diagnosing dermatofibroma visually with no kind of test? Has anyone had anything similar? And I read there are even differnt types of Dermatofibroma! I also had pre existing pre cancerous cervical cells (told 6 yrs ago) that I had checked during the pap and am awaiting results (I dont think I have anything just being cautious at this point) if the info helps. I need HELP! Another opinion from someone who is experienced in a field or from personal experience. Melanoma is potentially fatal. I feel like there is a spider on me that is poisonous at full grown and the dr's are saying wait till the baby gets ready to bite. I dont want to wait for the poisonous spider to bite me, ya know? Squash it before it does in case the treatment dont make it before the poison takes over. HELP!!!
** and as for the vaginal bump the gynecologist confirms the lump is not a result of an std etc (My BF is soooooo lucky or he'd pay!)

Answer: Go ahead and have them surgically removed and than they can be biopsied so you will know exactly what you are dealing with. As for your worries about insurance you can keep your insurance but you will need to pay for it yourself out of pocket . . which if it turns out to be malignant would be a good thing . . not really sure what your problem is here other than being frustrated with the Dermatologist. Dermatologists can make mistakes too since I know a young neighbor who was told there was no problem with a mole but had it removed anyway and the biopsy revealed a malignancy (it was melanoma). Her doctor apologized. So Dermatologists can be wrong.

Answer: Tonometry.....it measures pressure in the eye.
Question: Flashes of Light, Objects and Other Weird Stuff? Hi All,
I've been having this weird things in my vision.
They are usually flashes of light in the periphery (sometimes in the central) part of my vision. I also see shadows/objects that pass by really fast in the periphery. This has been going on for a couple of weeks now.
I've been to an ophthalmologist, and had a slit lamp exam, visual field testing and OCT. Results from the visual field test are above normal on my left and right eye. The OCT exam revealed a healthy retina on both left and right eye. The only problem is I have a slight pressure in my optic disk in the right eye. I went back to the ophthalmologist, and she said the pressure on the optic disk in my right eye is nothing to worry about, but just in case I have to go back for another eye exam after 3 to 4 months.
The flashes of light and shadows/objects that pass by in my periphery she can't explain.
I also have myopia since my preteens.
Hope the information above can shed some light as to what I am experiencing.
I also have this when I blink.
I also am not into any type of drug/s since my teens.
I've had a dilated exam on both my eyes and their fine. I also have a very healthy and normal macula.
Also about the floaters, I've already had one in my right eye since I was a kid. I've also a few 'transparent' ones that I only see when looking at a white background or the sky. this 'transparent' floaters have been in my field of vision since I was a kid.
Answer: Sounds like your eyes' vitreous is going through changes.

Don't be surprised if these floaters move into the foreground, more central area, of your vision.
If I were you I'd make sure I get a dilated exam of my retinas again in a couple of weeks...
Question: please tell me what these tests are? i've got to see the eye dr. again. he ordered more tests for my eyes. can anyone please tell me what the following is for and what happens during the tests? i'm including the codes for each test.

Answer: 92014 is the code for level of service. Just means established patient eye exam.
92135 rt/lt means Optical Coherence Tomography, which is a picture taken of your optic nerve to check for damage due to glaucoma.
92020 means gonioscopy which is where the doctor measures the angle structure of your eyes to insure that they are not too narrow which can cause high pressure spikes.
76514 means pachymetry. It measures the central thickness of your cornea.
92083 means the visual field test where you pushed the button when you saw the flash of light.

I think that the dorsal (posterior) horn and the dorsal (posterior) root ganglion are comprised of sensory axons only and that the ventral (anterior) root was comprised of motor axons. I wasn't sure of the lateral funiculus so I put answer 5. None of the above.
The next three questions I am pretty sure of, but would appreciate it if someone could confirm my answers.

I believe the answer is 5. Two of the above because I think the Hypothalamus and the Thalamus are both affected in people who have sleeping disorders such as Narcolepsy.
E. If you were to perform a spinal tap (inserting a needle into the spinal cavity to extract some CSF), where would be the best place to do it in order to reduce the chance that you may cause nerve damage?
1. In the region with the cauda equina

I think that the answer is 1. In the region with the cauda equina because the safest place for a spinal tap is in the lumbar region. Specifically, in the lumbar vertebrae L3-L5 where the cauda equina is primarily located while the conus medularis is located primarily in the lumbar vertebrae L1-L2.

Question: Seeing Halo's and Starbursts -- long term? 30yr old.? For the past 2yrs I have been seeing Halo's and Starbursts around lights.. whether it be a street light, or car headlight.. or even the little light on my cable box. They didn't used to be there... I have other anomolies such as patterns seem to move (persian rug patterns) or shutters with sunlight on them from a distance. (More of a rolling pattern.. without sunlight beaming on them they seem fine).
I am 30yrs old, and have been to the optometrist recently and had a report of visual acuity of 20/15 -- perfect visual field test. -- And a slight astigmatism of .25 diopters.
From my understanding the .25 astig is the least amount possible and is quite common, and really not enough for correction. To note: She tried various lenses and prisms in the machine but nothing corrected any of these things.
Anyone care to suggest what might be going on?
It was a dilated exam and the eye appears to be healthy. What is being missed here?

Answer: since dillated fundux examination reveals normal, and such a small astigmatism, other thing i can think of is...aging lens? but you are not tat old either...
but one thing you may consider, get some glasses with anti-reflection coating, it normally reduces the starburst effect.
Question: blind spot appeared in vision? about 2 weeks ago I noticed a spot in my vision of my left eye. This spot did not seem like a floater and it did not drift around. It is located a little below my central vision.
I saw an Opthamalogist 2 days ago and he did a thourough exam and dialtion. He found nothing abnormal, and he told me it was a floater. I asked him if a floater could stay in the same spot in my visual field and he said yes. He also mentioned it could be caused by a migraine, but the symptoms have not gone away.
The spot is not as noticable as it was when it first appeared - however I cannot see through the spot at all - it has turned into a blind spot.
I am extremely comcerned about this. I am 28 yrs old. What should I do about this? Will it get worse?

Question: Floaters reflecting light, need help? I first noticed floaters in my eyes in my early 20's. I immediately had my eyes examined and was subsequently giving a clean bill of ocular health. Well within the past 6 months I've been having several different strange happenings. For a short time I was noticing a sharp dot appearing flashing, sometimes it was blue, sometimes white, always in the same area and only lasting milliseconds. I had my eyes examined, dialted and everything and was given a clean bill again. Now I am noting a dot in the same area but this time it's black, and now my floaters appear to be "bending" or "refracting" the backlight of my computer. Now several times an hour I notice these sparkles floating across my vision (I can see from their movement they must be floaters). I had my eyes examined a few weeks ago and will go yet again for a field exam but I'm at a loss.
I was always told seeing things such as this was a serious sign of visual health. Can floaters worsen and capture and bend light? Why would a sharp, clear, colored dot keep randomly appearing?
I'm so, so lost.
Seeing these things all day is getting really distracting for me and I don't know what else to do. Should I keep getting different opinions?

Answer: I recently had an eye exam and told the doctor that my floaters seem to be getting worse. He said that everyone has floaters, and they increase in frequency as we age. They are much more noticeable when looking into bright sunlight.
Generally speaking, they are nothing to be alarmed about and are not indicators of something wrong. But, the dot that you are seeing is something I have not experienced.
Have you been seeing an optometrist or an ophthalmologist? An ophthalmologist is a medical doctor who specializes in the eye. I would say that since you are having problems, you should choose an ophthalmologist over an optometrist.
Question: Glaucoma for someone whose 43? Just had my annual eye exam, optometrist said my left eye which is a lazy eye, the top fiber optic nerve looked thinner then last year, so I went to an ophthalmologist today, who looked, took a picture and there is a change, however he said it could be normal for me or it could be an issue, a possibility of glaucoma but it's rare for someone my age, this doesn't un on my mothers side of the family and I never knew my father, so I don't know, all I know is that I could go blind and I would honestly rather kill myself if I do have it rather then wait around for the ball to drop, I'm already not working, only temping and now this, to many bad things have happened to me, god my really hate me, I go in for a visual field test on Mon which he said there is a learning curve to the test and my girlfriend whose an ophthalmic tech said the same thing, that it may take a few try's but I want to know if anyone who is in my age group or even younger has this and for how long, how are you coping, I just don't think I'll make it to Mon., I just can't live this way, I can't. I would even love to meet with you to discuss this in person, I live in the San Francisco Bay Area along the peninsula. Thank you.

its really not that bad.
take a study dose of ibuprofen until monday and put a warm wash cloth on your eye to help ease the pain.
i know it can be painful but its nothing to be this upset over.
after they figure out how bad the glaucoma is they will decide how to treat it. i had 2 different treatments cuz i had it in both eyes and one worked for the one eye and not the other.
they first try an eye drop. mine is called cosopt. just put it in as many times as they say per day (mine is 2 times per day) and hopefully it will control it. now this is not going to cure it you would need to take the eye drop everyday for the rest of you life but i've been taking it for 10 years now and its more or less just built into my brain now. i don't even have to think about it.
the other treatment i had was they put a tube in my eye so that the fluid would drain how it is supposed to. my eye was sore for a few days but after that it was fine and i haven't had an issue with glaucoma in my eye since and that was 7 years ago. and that can be removed incase your pressure gets too low or the tube gets blocked.
i wouldn't suggest a "freezing" treatment cuz once they freeze some of the cells they can't come back so if they freeze too many the pressure in your eye can drop too low and your eye can literally deflate and you can loose it.
but really i think you need to take a step back and calm yourself. hang in there until monday when they can help you.

Question: Visual disturbances + sick symptoms... any ideas? I'm a 26 year old male in the northeastern USA. For the past 10 months I have had the following symptoms, most of which have gradually been getting worse:
(1) flickering / spotty / pulsating vision - i.e. looking at a white wall gives me mild moving flickering dark spots, looking at a dark wall gives me the same except the spots are light. these are not floaters - their edges are very indistinct and i can see through them very easily. this is more aggravating when i am trying to read text, but it is always there.
(2) a more constant dark spot in the very right side of my left eye's field of vision
(3) occasional ghosted text in both eyes (intermittent bilateral monocular diplopia??
(4) occasional blurry vision
(5) frequent general out-of-it feeling: i feel tired all the time, have constant mild cold symptoms, occasionally feel disoriented, lightheaded, nauseous, or have an upset stomach.
(6) frequent hives
here's what i've done so far to check:
(1) GP has standard bloodwork done and resting EKG done. everything is fine.
(2) Ophthalmologist examines eyes, says everything is fine, implies that maybe something is wrong with my brain.
(3) Neurologist does exam and MRI of brain, says everything is fine.
(4) Neuro-ophthalmologist does eye exam, says everything is fine. Tells me I should see a psychiatrist.
(5) Allergist cannot explain hives - allergy skin test is negative
My questions and insights:
(1) Could this really all be stress / hypochondria? If so, what can I do about it?
(2) Over the past five years I have had unprotected oral sex with about 4 different people. Could this somehow be HIV or some other STD?
(3) I've gotten sick this year more than normal. Six months ago I had the worst diarrhea of my life, and three months ago I had a week of shivering and night sweats, which I've also never had before. I assume this was just a nasty flu - but could this all come down to some disease like Lyme Disease, Diabetes, or Encephalitis? Would my normal blood work have indicated any of this?
(4) Should I go back to the neurologist and insist on getting an EEG to rule out other brain-related things? Should I repeat my MRI?
(5) I've also had a sore neck for the past 5 years and occasional unexplained chest pains (doctor says it's just stress). Could all of the vision issues somehow be related to disrupted blood flow to the brain or pinched nerves in the neck?

Answer: Go back to GP, and have test for two things. SED rate. Complete white blood count (you need to know eosinophil level) and you want a referral to rheumatology.
These are classic, actually textbook signs of vasculitis. Have these tests done when you feel at your worst.

i want to be able to update ALL the student ID field in all the tables I mentioned above whenever i add an entry to the student ID field in any table.
I have connected every table by establishing relationships among them. Will those be helpful? thanks.

oh i got it! yeeey! please don't mind the additional details i mentioned earlier...
Answer: Yes. Creating relationships will make sure that when you enter information in a form it will make all the related fields the same.
If you change the student ID on an existing record you will either need to program the related tables to update or turn on cascading update in your relationship. Cascading update makes sure that anytime you change the related field is changes all fields that share the relationship.
Question: What should the doctor do? The Case of Mary Jane
Mary Jane(not her real name) is a 22 year old female who originally presented to neurology department at Mt. Sinai Hospital in NYC with symptoms that included hearing loss, problems with equilibrium, and general acoustic disturbances. She was given a c.a.t. scan (Computed Tomography scan) which uncovered a number of what turned out to be benign tumors which gave her a diagnosis of Neurofibromatosis, also know as the ‘elephant man’s disease’ ( a genetic condition that cause benign tumors to develop on each nerve ending (usually the teleodendrite)). While the illness is not itself life threatening, since the tumors continue to grow if left alone they will eventually impinge on the brain itself and cause a slow and painful death.
After consulting with the department of Neurosurgery she agreed to have the tumors removed from her auditory pathway. This meant however that she would loose hearing in both ears. The operation was a success and she was discharged with an Rx to follow up with the department every six months to monitor her condition.
On her third visit she complained about visual field disturbances in her vision. She was experiencing bright flashes and loss of peripheral vision in both eyes. She was admitted to the hospital and given a series of evoke potential exams (low level electrical currents are monitored via an E.E.G. (electroencephalogram) and was found to have additional begin tumors on her visual pathway. The obvious course of treatment would be to ‘harvest’ these tumors as well and this was the course of treatment presented to her.
Upon questioning the neurosurgeon Mary Jane found out that in fact her illness would continue unabated since there is no treatment for Neurofibromatosis and that she would continue to loose functionality as the disease progressed. Mary Jane was fully aware what this meant: she would eventually die from a tumor that was lodged in an inoperable area.
I was consulted by the chief of neurosurgery and asked to help him make a decision about what to do. Mary Jane was originally opposed to any more operations on the grounds that since she would die anyway she would like to keep her vision until that time. With no treatment and no hope for the surgery to eventually remove all her tumors she felt strongly that she would like to die with as many of her senses as possible.
The surgeon knew that he had some influence over her and that if he pushed hard enough, with both Mary Jane and her family, he could probably get permission to ‘harvest’ these tumors.
The question is should he apply pressure to convince Mary Jane to accept the operation or should he stand by and watch her die when the operation would save her life at least this time.
Answer: Personally, I'd let her live and die as she pleased. It's almost like keeping her in a semi-vegetative state against her will to try and force her to get the operations. Not cool. Or, you could say just as you can't take a person's life against their will, you can't save it either(but that's getting into muddy waters when you think about all the perfectly healthy people who loose the will to live.)
Plus, I don't blame her for that decision; I'd want the same if it were me or one of my loved ones.

In the childhood days I am very passion towards computer, I used to play many games, Paint with the help of the computer. I scored 100 percent in my computer science exam which mainly deals with basics. I thought it’s a magical machine but after matured I came to know it is a machine which contains set of program written by the programmer.
In the higher secondary level I came to know the various parts of the computer, once I saw the mother board it consists of chips, but I don’t know its working in those days, I asked many doubts to my brother and teacher who clearly explained all the parts of the computer. Then I am very much familiar with all the internal parts of the personal computer, it also helps to develop more interest on computer science field.
I like to develop my knowledge more into that field, so I preferred B.E computer science and Engineering from the breadth of Anna University. From the lecture of efficient professor I came to know various internal operations of the computer like program, operating system, etc once I entered in the second year I know the various details of the computer science application. I learned more subjects which includes languages like c, oops concepts, DBMS, OS, Networks, etc., so I am very much clear with the basic concepts of these subjects I like to enhance my knowledge in this particular field by pursuing MSc in Advance Computer Science from your Institution. I believe it will also serve to give direction to my goal of a career as a research professional at an academic or commercial, research-oriented organization.
In my undergraduate studies, I attained information from the breadth of Anna University’s syllabi content that has given me a complete exposure to the heart areas of Computer Science and a strong conceptual understanding of the same. In those four years of study, I strived to maintain an approach of expending independent effort in all my deeds. Learning, putting them to practice and sharing my knowledge with others has been most valuable and takes pleasure in, when comprehending a concept.
Over these years, I expanded the knowledge in all the areas of Programming Concepts in Computer Science. Thereafter, to automate the ideas, I started gaining knowledge in various programming languages like C, C++, JAVA, Visual Programming, J2EE, mobile technologies like J2ME, WAP, WML, backend databases like SQL Server, Oracle and Scripting languages like JavaScript, VBScript, in college. In the third year I develop my self in various fields of computer science and I develop a program which is utilized for practical works, I gained affection from my professor and they motivated a lot, which gives me more passion towards that particular field, By gaining affection and attraction from my staffs and friend I was elected as the president of computer science department and organized International Technical Symposium. I was the coordinator for PARROT club (student’s organization) organizing technical college events, and magazine releases. I won third Prize in Dance competition in IIT Sarang.
For my B.E project, I decided to concentrate on mobile based mapping application based on J2ME Technology. The J2ME application living on the cell phone was used to communicate with the remote systems. Locations are expressed both as graphical and textual descriptions. The project examines the best way to allow real time, read only access, for remote, possibly non-technical users, to an information system. I also done a project PAWS that supports both process design and execution. It supports the aspects of adaptation, coupling design-time and runtime mechanisms in a global environment. An website was created for a an institution where each student and staff were provided a webpage and options available to design their webpage like the background color, upload picture, add templates etc. .I also underwent a 15-day training in BSNL.
My future plan is to develop a new IT Sector in most innovation and advancing concept, this can be obtained by studying the concepts thoroughly which helps to develop my skills in that particular field, this can be obtained from post graduation and it is worth by doing Msc in Advance Computer science from your institution.

Question: Which high school/university courses will I need to take to become a forensic scientist? I am in year 8 and need to start choosing electives at mid/end of year for year 9. I live in australia, if that matters, and I need to know which senior (year 11 and 12) courses I will need to take in order to qualify for entrance exams to the university and university courses I will need in order to be a forensic scientist.
So, from experience what courses will I need for these grades:

Answer: To get into uni you only need the SCORE required. The actual subjects you take are irrelevant. So dont worry about what subject to take in year 11/12.
Now forensics is not what is seen on TV. There are limited number of jobs, depending on the state they are either associated with the police department, hospital or private contractor. When I say limited I mean 1-3 jobs per year open up. Imagine the competition! For managerial positions you will need an MBA and PhD most likely. For lab teach a TAFE degree is in most cases more preferable than a uni degree. That is because lab work in a forensics lab is like in a hospital lab - and a Diploma in Laboratory Science is what they are after.
University science is mostly theory with only a small number of practicals whereas TAFE science is all practical application and small amounts of theory. In a forensics lab - the managers/bosses do the thinking and the lab techs do the actual lab work - they need to be able to do the same experience 2 times a day every day of the year exactly in the same way. That is what they are trained to do.
